JIFSAN Internship Program

• Procedures for new JIFSAN interns (paperwork that must be completed before your internship can begin)
• Volunteer Commitment Verification form (to keep track of the hours you have worked during your initial, volunteer semester in the JIFSAN program)
• Paid Internship form (completed each semester for continuing students and new students who have fulfilled their volunteer requirement)
• Immunization Payment Request (for students that need immunizations form the Campus Health Center prior to beginning their JIFSAN internships)

• W-4 Tax Withholding Certificate (required) and instructions (Please note that if you reside in the District of Columbia, a different form is required)
• I-9 Employment Eligibility Verification (required)
• Citizenship Status Form (required for anyone who is not a US citizen, including Permanent Residents)
